Referees are required to fill out an incident report whenever they issue a red card. They are also required to submit a report if they encounter anyone (parent, coach, player, referee) who breaches the Metro League code of conduct. All incident reports should be submitted to metroleaguereferee@gmail.com within 48 hours of the incident so that they can be promptly addressed.
If you issued a red or yellow card during a game you must complete the following form. Once this form is completed you must e-mail the form to metroleaguereferee@gmail.com within 48 hours of the game so the issue can be promptly addressed. Please note that if you issued a red card, you must also fill out the incident report form that is posted above.
